UX improvement to the self assessment page

This is a wrapper issue for an initial pass on improvements to the UX of the self assessment page.

Suggested improvements

• Include a contents at the top or in the sidebar to quickly jump to a section
• Include search functionality as described in Skill form filtering #676
• Show or link to information on the levels

Additional context
These are driven by requirements to support N8 interns completing the form.

I think the self-assessment page is going to be significantly overviewed. My thoughts are this:

• Move to a paginated view where each Competency is its own page. Kind of like a questionnaire or application form. These can be sequential, or maybe more flexible, like being in tabs.
• Reduce the number of skills that users need to fill out by selecting a Role (see Create Role and RoleFamily models #603 and Role and user persona support #747)
