chore(beads): lead bd-5n7ks2 with the mechanism-not-care argument

The bead now opens with the fact that carries it: a fresh instance of the
pattern was produced INSIDE the artifact documenting the pattern — '~40 scripts
lack pipefail' came from reading the first 20 lines of an rg listing as the
total, written into the bead about truncated views reported as totals, by
someone who had just audited the fleet for that exact trap.

Care is not a control; it is the thing that fails. Stated first because the
counterargument is always that people should be more careful.
